The Restorative Care Program
at Founders Pavilion

OVERVIEW

Corning Hospital's 120 bed nursing facility, Founders Pavilion, operates under a residential health care facility license in providing skilled, long term care and provides comprehensive inpatient rehabilitative care.

After full initiation of the rehabilitation program in 1994, it's success continues to be illustrated by the positive "outcomes" of residents admitted and ultimately discharged successfully to independent living. In 1995, 1996 and 1997, we successfully discharged to home more than 50 residents per year.

 

HOW LONG WILL I BE THERE?
The length of stay in the rehabilitation program varies from as short as five (5) days to as long as many months of rehabilitation prior to discharge home. The length of stay that you may experience will vary from individual to individual, of course, depending on the extent of your injuries and/or degree of debilitation.

WHO WILL CARE FOR ME?

The program employs a full complement of rehabilitative staff including: Physical Therapists (PT), Occupational Therapists (OT), Speech Therapists (ST), PT Assistants, Certified OT Assistants, as well as three (3) nurses. The Activities Department provides yet another aspect to the rehabilitation service, with the availability of a full time Recreational Therapist.

The rehabilitation staff, in cooperation with a multidisciplinary team focused on rehabilitation, maps out a plan of care and service for each resident to ultimately return them to independent living as soon as possible.

WHAT ABOUT QUALITY?

Our nursing facility is fully licensed by the New York State Department of Health (DOH) and achieved a "no deficiency" rating on our 1996 survey which ranks us in the top 22 percent nationwide. We are also accredited by the Joint Commission on Accreditation of Health Care Organizations (JCAHO) which promotes us as one of only 1200 facilities across the country that have received accreditation.

HOW CAN I GET INTO THE PROGRAM?

If you think you are a candidate for admission into the rehabilitation program, ask your Attending Physician, Social Worker, and/or Discharge Planner.

HOW MUCH DOES IT COST?

The actual cost to you personally will vary depending on your insurance coverage and your eligibility status for Medicare Part A and/or Medicaid. All financial arrangements will bel made prior to admission, and we will provide all necessary assistance and counseling regarding the payment arrangements available to you.
